Any perpetrator of child abuse or neglect on the Child Abuse
and Neglect Central Registry may apply in writing to the Secretary to have the
perpetrator's name expunged from the central registry when three years have
passed since the most recent confirmed incident.

include your full name (first, middle, last), date of birth, and if known,
names of children involved.
